大株红景天注射液联合贝那普利治疗高血压伴心力衰竭的疗效及对血管活性物质的影响

摘要:

目的 探讨大株红景天注射液联合贝那普利治疗高血压伴心力衰竭的疗效及对血管活性物 质的影响。方法 选择2016年1月至2018年9月于郑州大学附属郑州中心医院接诊的80例高血压伴心力衰 竭患者进行研究,随机分为观察组和对照组,每组各40例,在常规抗心衰基础上,对照组给予贝那普 利治疗,观察组在对照组基础上联合大株红景天注射液,均连续治疗14 d。比较两组临床疗效、血管活 性物质、血压、心功能指标的变化及不良反应。结果 观察组临床疗效明显高于对照组(P<0.05); 血管活性物质中,观察组内皮素-1(ET-1)、血栓素2(TXB2)明显低于对照组,6-酮-前列腺素F1α (6-keto-PGF1α)、一氧化氮(NO)明显高于对照组(P<0.05);观察组收缩压(SBP)、舒张压 (DBP)明显低于对照组(P<0.05);心功能指标中,观察组左室射血分数(LVEF)明显高于对照 组,左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)明显低于对照组,6 min步行距离明显 优于对照组(P<0.05);两组患者不良反应总发生率差异无统计学意义(P>0.05)。结论 大株红景天 注射液联合贝那普利治疗高血压伴心力衰竭效果较好,可有效调节血管活性物质表达,积极降压、改善 心功能,安全性高,值得推广。

Abstract:

Objective To investigate the curative effect of Dazhu Hongjingtian Injection combined with benazepril and its influence on vasoactive substances in patients with hypertension complicated by heart failure (HF). Methods The patients with hypertension complicated by HF (n=80) were chosen from Zhengzhou Central Hospital affiliated to Zhengzhou University from Jan. 2016 to Sept. 2018, and divided randomly into observation group and control group (each n=40). On the routine anti-HF therapy, control group was given benazepril and observation group was given Dazhu Hongjingtian Injection combined with benazepril for 14 d. The clinical efficacy, level of vasoactive substances, blood press (BP), changes of heart functional indexes and adverse reactions were compared between 2 groups. Results The clinical efficacy was significantly higher in observation group than that in control group (P<0.05). As to vasoactive substances, the levels of endothelin-1 (ET-1) and thromboxane 2 (TX2) were significantly lower, and levels of 6-keto-PGF1α and nitric oxide (NO) were significantly higher in observation group than those in control group (P<0.05). The level of systolic blood pressure (SBP) and diastolic blood pressure (DBP) were significantly lower in observation group than those in control group (P<0.05). As to heart functional indexes, left ventricular ejection fraction (LVEF) was significantly higher, left ventricular end-diastolic inner diameter (LVEDd) and left ventricular end-systolic diameter (LVESd) were significantly lower, and 6-minute walk distance (6MWD) was significantly higher in observation group than those in control group (P<0.05). The different in the total incidence rate of adverse reactions had no statistical significance between 2 groups (P>0.05). Conclusion Dazhu Hongjingtian Injection combined with benazepril has higher curative effect on hypertension complicated by HF, which can regulate effectively the expressions of vasoactive substances, reduce BP actively, improve heart function and has higher safety.
