急性心肌梗死患者血清中miR-133a、miR-133b表达与预后关系

摘要:

目的 探究急性心肌梗死患者(AMI)血清中miR-133a、miR-133b表达与患者预后关 系。方法 选取辽宁省丹东市第一医院于2016年5月至2017年11月收治的96例ST段抬高型AMI患者,另 选取同期本院健康体检者96例为对照组。AMI患者入院后12 h行急诊经皮冠状动脉介入治疗(PCI)。 随访半年内心血管不良事件的发生情况;使用超声心动图检测PCI后半年左心功能左心室舒张末期容积 (LVEDV)、左心室射血分数(LVEF)和左心室收缩末期容积(LVESV)变化;利用荧光实时定量 PCR(qRT-PCR)对血清中miR-133a和miR-133b水平进行检测;并分析miRNAs水平与PCI后半年左心 功能变化和心血管事件发生关系。结果 入院时AMI患者血清miR-133a和miR-133b水平比对照组较高 (P<0.05)。与miR-133a低表达组相比,miR-133a高表达组PCI后半年LVEF较高,LVEDV、LVESV较 低(P<0.05);与miR-133b低表达组相比,miR-133b高表达组PCI后半年LVEF较高,LVEDV、LVESV 较低(P<0.05)。miR-133a高表达组PCI后半年心血管事件高于miR-133a低表达组(P<0.05),miR- 133b高表达组PCI后半年心血管事件高于miR-133b低表达组(P<0.05)。结论 AMI患者血清miR-133a、 miR-133b水平上调,与PCI后半年左心功能和心血管事件有关,可能评估AMI患者预后。

Abstract:

Objective To investigate the relationship between miR-133a and miR-133b expression in patients with acute myocardial infarction (AMI) and their prognosis. Methods Ninety-six patients with ST-segment elevation AMI treated from May 2016 to November 2017 in Dandong First Hospital of Liaoning Province were selected as the control group. Patients with AMI underwent emergency percutaneous coronary intervention (PCI) 12 hours after admission. The incidence of cardiovascular adverse events within six months of follow-up; the changes in left ventricular end-diastolic volume (LVEDV), left ventricular ejection fraction (LVEF), and left ventricular endsystolic volume (LVESV) were measured using echocardiography during the six months after PCI The levels of miR- 133a and miR-133b in serum were detected by fluorescent real-time quantitative PCR (qRT-PCR); the relationship between miRNAs and changes in left ventricular function and cardiovascular events after six months after PCI was analyzed. Results Serum miR-133a and miR-133b levels in AMI patients were higher than those in the control group at admission (P<0.05). Compared with miR-133a low expression group, miR-133a high expression group had higher LVEF, lower LVEDV and LVESV six months after PCI (P<0.05); compared with miR-133b low expression group, miR-133b high expression In the group, LVEF was higher, and LVEDV and LVESV were lower after half a year after PCI (P<0.05). Cardiovascular events in the miR-133a high-expression group after six months after PCI were higher than those in the miR-133a low-expression group (P<0.05), and miR-133b high-expression groups had higher cardiovascular events after PCI in the first half year after PCI (P<0.05). Conclusions The serum levels of miR-133a and miR-133b in patients with AMI are up-regulated, which is related to left ventricular function and cardiovascular events six months after PCI, which may assess the prognosis of AMI patients.
